Agreement

I acknowledge and accept the responsibilities associated with my participation as a representative of go MLR in the Nurse Con '24 event in Orlando, FL. I am dedicated to upholding the highest standards of appearance and professionalism, strictly adhering to the event's schedule. Timeliness is crucial, and I will promptly notify the management team in case of any delays. Hairstylists and Barbers are required to possess a valid license for operating hot tools and providing haircuts. Makeup Artists will apply camera-ready makeup using disposable or easily sanitized products. Photographers/Videographers will deliver high-resolution assets for client/attendees, content creation and related promotional materials. All team members must perform effectively under pressure and maintain strict sanitation practices. All contractors participating in the event are required to possess a valid Certificate of Insurance (COI) with coverage in effect at the time of the event. Additionally, the COI must list go MLR, LLC as additionally insured, with the designated address at 7 Avenue de Lafayette, Boston, MA 02111 Team Roles + Hours 1X Photographer 3X Makeup Artists 3X Hair/Barber 1X Production Assistant Total: 18 Hours (Subject to change) In consideration of my services, I acknowledge the compensation of $30 per hour, tips, shared overnight accommodations, air/ground transportation, and meals/snacks during duty hours. Payment will be issued within 90 business days from the event booking date through Square's direct deposit system or bank wire transfer. Approved expenses for baggage, ride-share, and related items must be submitted for reimbursement within 5 business days through the provided portal. I understand that all terms and conditions outlined in the Contractor Agreement are applicable to my participation in the Nurse Con '24 event. Furthermore, I recognize that event details, including hours of operation, may be subject to change or cancellation due to unforeseen circumstances. Any inquiries can be directed to info@gomlr.com.
