Contractor Acknowledgment and Agreement – Fanatics Event NYC I acknowledge and accept the responsibilities associated with my participation as a contracted representative of go MLR, LLC (“the Agency”) for the Fanatics event taking place in New York City. By signing below, I agree to comply with all professional, legal, and safety requirements outlined herein and in the go MLR Contractor Agreement. Professional Standards + Conduct All contractors are expected to maintain the highest standards of personal appearance, professionalism, and hospitality. You are required to: Arrive on time and be ready to perform at your scheduled start time. Tardiness may result in removal from the assignment and forfeiture of compensation. Notify the go MLR management team immediately in the event of a delay or emergency. Maintain a professional demeanor with clients, staff, and attendees at all times. Licensing + Insurance Requirements Barbers and Nail Technicians must hold valid professional licenses required to operate in the state of New York. All tools and products must meet current health and sanitation codes. Photographers/Videographers are expected to deliver high-resolution digital assets suitable for brand content and marketing use. Ownership of content remains with go MLR unless otherwise agreed in writing. All contractors must carry a valid Certificate of Insurance (COI) with coverage in effect during the event. The COI must: List go MLR, LLC as an additional insured party. Reference the agency’s address: 7 Avenue de Lafayette, Boston, MA 02111. Be submitted prior to the start of the event. Failure to comply may result in cancellation of your booking. Event Assignment & Staffing You are being contracted under the following assignment: Team Roles: • 5x Barbers/Groomers • 4x Nail Technicians • 3x Production Assistants • 8x Brand Ambassadors Billable Service Hours: Up to 17 hours (subject to change at the discretion of the client or agency) Compensation + Payment Terms Rate: $30/hour, plus tips (where applicable) Parking and meals are not included. Please plan accordingly. Payment will be remitted within 60–90 business days from the date of the event, via bank wire or approved invoice. Submit invoices immediately after the event to: submission@gomlr.com. Include full banking details. Do not contact go MLR regarding payment before 60 business days have passed. Approved expenses must be submitted via the expense portal within 5 business days post-event. Late submissions will not be reimbursed. Additional Terms All contractors are bound by the general Contractor Agreement signed with go MLR. This includes but is not limited to: confidentiality, content usage rights, conduct policies, and non-compete clauses. Assignments may be modified, rescheduled, or canceled due to weather, client direction, safety concerns, or other factors beyond go MLR’s control. In such cases, contractors will be notified as early as possible. Any failure to comply with the terms above may result in removal from the event and/or termination of future bookings.
