Contractor Agreement

I acknowledge and accept the responsibilities associated with my participation as a representative of go MLR at the TX Conference for Women event in Austin, TX. I am dedicated to upholding the highest standards of appearance and professionalism, strictly adhering to the event's schedule. Timeliness is crucial, and I will promptly notify the management team in case of any delays. Hairstylists are required to possess a valid license for operating hot tools. Makeup Artists will apply camera-ready makeup using disposable or easily sanitized products. Photographers/Videographers will deliver high-resolution assets for client/attendees. Images are to be delivered within 3 business days via Pixieset gallery. All team members must perform effectively under pressure and maintain strict sanitation practices. All contractors participating in the event are required to possess a valid Certificate of Insurance (COI) with coverage in effect at the time of the event. Additionally, the COI must list go MLR, LLC as additionally insured, with the designated address at 7 Avenue de Lafayette, Boston, MA 02111 Team Roles + Hours 2X Photographers 1X Production Assistant 1X Makeup Artist 1X Hairstylist In consideration of my services, I acknowledge the compensation of $75 (Hair + Makeup Artists) + $150 (Photographers) per hour + parking. Payment will be issued within 60-90 business days from the event booking date via bank wire transfer or invoice. Approved expenses must be submitted for reimbursement within 5 business days through the provided portal. Direct invoices with banking instructions and W-9 form following the event to: Submission@gomlr.com I understand that all terms and conditions outlined in the Contractor Agreement are applicable to my participation in the TX Conference Women event.
